Sponsorship Options - LHLL offers many levels of sponsorship as outlined on the sponsorship form below.  We can customize based on the level of participation that fits your organization.  Please don't hesitate to contact us with questions.

Injury Tracking Forms - An injury requiring ANY medical attention should be reported to the League Safety Officer (refer to the current listing of board officers via the 'Contact Us' page). Only the League Safety Officer or the League President should report an accident or injury to District 55.

Little League Volunteer Application - Laguna Hills Little League is a volunteer organization, and as such we require our families to volunteer in many ways that improve the experience that our children receive from their time in Little League.  If you will be on the field (even during practices), in the dug-out or volunteering at skills clinics or in the concession stand, you will need to REQUEST AN ONLINE VOLUNTEER APPLICATION from the League Safety Officer.  A  color copy of a state ID or drivers license will be required at verification.  We background check every volunteer as a safety precaution.

Little League Proof of Age & Residency Requirements - Each local Little League® determines the actual geographic boundaries of the area from within which it shall select players. These boundaries must be described in detail and shown on a map and dated when making application for a Little League charter. Players will be eligible to play with that league only if they reside or the physical location of the school where they attend classes is within the boundaries provided to and approved by Little League Baseball®, Incorporated.

2024 - LHLL Safety Plan 

Parent Code of Conduct Waiver -  This document outlines the proper role of parents in supporting their child in Little League and must be signed and returned along with all other registration documents at verification prior to every season.

Medical Release Form - To be carried by any Regular Season or Tournament Team Manager together with team roster or International Tournament affidavit.   This document must be filled out, signed and returned to a league official on document verification day before the start of each season.

Sudden Cardiac Arrest  - this information sheet needs to be reviewed with all players/parents, then signed and returned.

Concussion Training - All on-field volunteers and board members are required to take the Heads-Up concussion training course and provide a copy of the certificate to Laguna Hills Little League prior to the start of the season.  You will find all the information on the information sheet below.
